Chronic Care Guidelines

Objectives
Upon completion of this session, the learner will be able to:
- Recall the five phases of the Case Management Process
- Interpret the Indications for Chronic Care Management
- Use Assessment and Care Plan content to manage chronic care patients
- Recall the patient education resources included in the Chronic Care Guidelines
- Reference the Optimum Treatment Guidelines to support outpatient managment
Behavioral Health Guidelines

Upon completion of this session, the learner will be able to:
- Define the five levels of care within the Behavioral Health Guidelines
- Interpret the Clinical Indications for Admission for each level of care
- Apply key components of the Optimum Recovery Course to monitor inpatient progression
- Apply key component of the recovery course to monitor treatment progression at lower levels of care
- Define the use of the Behavioral Health Guidelines Annotated Bibliography
