This workshop is designed to provide maximum 'hands on' learning. Through the use of clinical cases, participants will learn to effectively and efficiently apply the guidelines to inpatient case management, concurrent review, discharge planning, outpatient case management, and pre-certification. New and experienced users alike will develop a solid working knowledge of the guideline products. Come learn new skills or sharpen the skills you already have. - Define the importance of optimal transitions from one level of care to another
- Describe the Care Guidelines’ inpatient discharge plan resources
- List four Care Management Tools pertinent to transitional care plans
- Identify three potential level of care transitions
Every day patients experience sub-optimal transitions from one level of care to another causing them confusion and fractured care. Every day payors and providers struggle to find the resources to ensure more seamless transitions. Every day patients, providers, and payors experience the ill-effects of this issue with readmissions, delays in appropriate care, and increased healthcare dollar utilization. On a national level, the National Transitions of Care Coalition (NTOCC) has recognized and is actively addressing this care management issue.
This session is designed to establish the depth and outcomes of this issue and to review the efforts to date of the NTOCC. The use of clinical vignettes will provide participants with the opportunity to collaboratively locate and utilize the Care Guidelines’ resources that support best-practice transitions across the care continuum.

There are a variety of Care Management/Case Management models being effectively utilized across the nation. This session is designed to define the impact of chronic disease and its current and expected impact on the health care delivery system. Does your organization recognize the importance of having a Care Guidelines subject matter expert (SME) on staff? No formal training department? Do you or your teammates frequently have questions about the guidelines that you wish someone was able to readily address? If you’ve answered "Yes" to these questions, then this workshop is for you!
This session will provide you with an in-depth exploration of the Care Guidelines products and best practices for their use in a variety of care management models and workflows. You will learn how to develop Care Guidelines-related job aids and will receive learning material and other Care Guidelines education resources to take back to work with you. The goal of this session is to send you back to your organization energized about the use of the Care Guidelines products, able to find answers to questions that come up, and prepared to help others learn how to effectively and efficiently use the products!
